The critical challenges in developing countries’ healthcare
Healthcare infrastructure in developing nations remains severely strained. Many countries lack adequate hospitals, clinics, and medical equipment, with rural areas particularly underserved. This makes it difficult for residents to access even basic healthcare services. The problem extends beyond infrastructure-there’s a critical shortage of trained healthcare professionals, and essential medical supplies are often unavailable.
Untimely disease detection
Early detection of diseases remains one of the biggest challenges. Without timely diagnosis, conditions that could be managed effectively become life-threatening. In developing countries, improved health outcomes often correlate more with access to healthcare knowledge and services than with income levels alone. However, diagnostic facilities are concentrated in urban centers, leaving vast rural populations without access to screening and early intervention services.
Inadequate diagnostic facilities and medical intervention
The distribution of healthcare resources shows stark inequalities. About 4.5 billion people globally were not fully covered by essential health services in 2021. The shortage extends to healthcare professionals, with doctors, nurses, and allied health workers concentrated in urban centers while rural and remote areas remain chronically underserved.
Financial barriers compound the problem. Only 17 percent of mothers and children in the poorest households in low-income countries receive basic maternal and child health interventions, compared to 74 percent in the wealthiest households. Information Technology (IT), Information and Communication Technologies (ICT), and Emerging Technologies (ETs) represent umbrella terms for digital innovations that can revolutionize healthcare delivery. The role of eHealth has been recognized as pivotal in attaining Universal Health Coverage and the Sustainable Development Goals.
Understanding the technology spectrum
Digital health encompasses the use of digital technologies to address health needs, with eHealth defined as the use of ICT in support of health and health-related fields. This includes mobile health (mHealth), which uses mobile wireless technologies for public health, as well as advanced computing sciences like big data analytics, genomics, and artificial intelligence.
Digital health encompasses technology including digital devices, software, and communication tools to enhance health and healthcare delivery. Applications range from monitoring and managing health conditions to diagnosing and treating diseases. Key components include mHealth apps, telehealth platforms, electronic health records (EHRs), wearable devices, and remote monitoring systems.
Achieving Universal Health Coverage and SDGs
Universal Health Coverage represents the aspiration that everyone should receive quality health services without financial hardship. Beyond health outcomes, UHC contributes to social inclusion, ending poverty, economic growth, and human dignity. Progress toward UHC is tracked through the Service Coverage Index and financial protection indicators.
Digital technologies played a central role in pandemic response and minimized social, economic, and health impacts. Leveraging technology can bridge the gaps toward UHC attainment in low and middle-income countries. India’s Ayushman Bharat Digital Mission provides one example of how digital health integration can support inter-sectoral coordination.
Implementing digital health for systemic change
The implementation of digital technologies in healthcare is broadly referred to as digital health. This field aims to transform traditional, hospital-centric care into a proactive, accessible, and patient-focused system.
E-health and electronic record management
WHO defines eHealth as the cost-effective and secure use of ICT in support of health and health-related fields. This includes healthcare services, health surveillance, health literature, and health education. Clear evidence exists on the growing impact of eHealth on healthcare delivery worldwide, making health systems more efficient and responsive to people’s needs.
Electronic health records (EHRs) serve as the backbone of digitized healthcare. They enable seamless information sharing between providers, reduce medical errors, and ensure continuity of care. For developing countries, transitioning to electronic record management means overcoming paper-based systems that are prone to loss, damage, and inefficiency.
Mobile health (mHealth)
mHealth has been shown to increase access to health information, services, and skills, while promoting positive changes in health behaviors and disease management. The ubiquity of mobile phones, even in remote areas, makes mHealth particularly powerful for reaching underserved populations.
Mobile applications can provide medication reminders, enable appointment scheduling, and deliver health education directly to users. mHealth applies smartphones and tablets to provide healthcare services with convenience, affordability, and real-time access for consumers. This enhances patient participation and treatment plan compliance while reducing the need for clinical visits.
Telemedicine and remote care
Telemedicine allows patients to consult with healthcare providers remotely, increasing convenience and access to care. Leveraging telemedicine and mobile health applications can expand healthcare access, especially in remote areas. Patients can receive consultations, second opinions, and follow-up care without traveling long distances to healthcare facilities.
In China, the Digital Health Kiosk program uses AI-enabled diagnostic support and telemedicine platforms to increase healthcare utilization while reducing out-of-pocket costs. Such initiatives demonstrate how digital solutions can address access disparities in rural populations.
Smart health and emerging technologies
Smart health represents the integration of emerging technologies like artificial intelligence, Internet of Things (IoT), and big data analytics into healthcare systems. Digital health innovations are transforming rare disease management by enhancing diagnostic capabilities, expanding access to specialized care, and accelerating research.
AI-powered tools can analyze medical images, predict disease outbreaks, and personalize treatment recommendations. Wearable devices enable continuous monitoring of vital signs, allowing early intervention before conditions become critical. These technologies shift healthcare from reactive to proactive models.
The path forward: integration and sustainability
To reach UHC goals, substantial public sector investment and accelerated action by governments and development partners are essential. Key actions include reorienting health systems toward primary healthcare approaches, advancing equity in access and financial protection, and investing in robust health information systems.
The maturity of the ecosystem-comprising enabling and ICT environments-critically influences the relevance and impact of digital health interventions. This includes leadership, governance mechanisms, regulatory frameworks, strategy, financial investment, workforce capacity, and standards for interoperability.
Success requires more than technology adoption. Reviews of health policies in developing countries advocate for increased investment in primary healthcare, capacity-building for healthcare professionals, and harnessing digital technologies to enhance healthcare delivery. Context-specific approaches that consider each country’s unique challenges remain essential.
Investments in health to support UHC boost economic growth through multiple pathways. Healthier workers contribute to increased economic productivity, with the estimated return on investment from improved health reaching nine to one in low and middle-income countries.
Overcoming implementation challenges
While digital health holds immense promise, implementation faces real obstacles. Infrastructure gaps, including unreliable internet connectivity and electricity in rural areas, limit technology adoption. Digital literacy varies widely, and healthcare workers need training to effectively use new systems.
Data security and privacy concerns must be addressed through robust regulatory frameworks. Telemedicine and eHealth have shown great potential in promoting health outcomes, but unintended consequences related to inequality risks require attention. Ensuring that digital solutions reach the most vulnerable rather than widening existing gaps remains a priority.
Sustainability requires moving beyond pilot projects to integrated national strategies. Public-private partnerships can bring innovation and resources to support public health goals. International aid and collaborations provide essential support, but long-term success depends on building local capacity and ownership.
