India faces a unique healthcare challenge: providing quality medical services to over 1.4 billion people spread across diverse geographies, from remote Himalayan villages to dense urban centers. Despite remarkable economic progress, the country has historically struggled with healthcare access gaps. Digital technology has emerged as a powerful solution, and over the past decade, the Government of India has launched an ambitious suite of e-health initiatives designed to transform how healthcare is delivered, accessed, and managed nationwide.

India’s healthcare landscape and the push for digital transformation

India’s healthcare system has long grappled with significant challenges including a shortage of medical professionals, uneven distribution of facilities between urban and rural areas, and fragmented health records. The National Health Policy 2017 emphasized implementing digital health across all levels of care to maintain continuity of patient care and improve functional efficiency. This policy laid the groundwork for what would become one of the world’s most ambitious digital health programs.

The foundations for India’s e-health journey were established through earlier initiatives like the National e-Governance Plan (NeGP) and the Digital India campaign. These programs created the essential digital infrastructure, including widespread internet connectivity, Aadhaar identification, and the Unified Payments Interface (UPI), that would later support healthcare digitization efforts.

The National Digital Health Blueprint and ABDM

In October 2019, the Ministry of Health and Family Welfare released the National Digital Health Blueprint (NDHB), providing an architectural framework for digital health interventions across India. This blueprint envisioned creating a National Digital Health Ecosystem supporting universal health coverage in an efficient, accessible, and secure manner.

Building on this foundation, the Ayushman Bharat Digital Mission (ABDM), initially launched as the National Digital Health Mission in August 2020, was rolled out nationwide in September 2021. The National Health Authority (NHA) is the apex body responsible for setting standards, implementing, and managing ABDM along with other public digital health programs.

Core components of ABDM

ABDM operates through several integrated building blocks. These include a unique health identifier for every individual, a personal health record system, DigiDoctor for unique doctor identification, and a health facility registry for standardized information about hospitals, clinics, laboratories, and pharmacies. The Ayushman Bharat Health Account (ABHA) provides each citizen with a 14-digit unique health identifier, enabling seamless access to their medical records across healthcare providers. As of December 2024, over 72 crore ABHA numbers have been created with more than 47 crore linked health records.

The evolution of telemedicine in India

India’s telemedicine journey has deep roots extending back to space technology applications. Between 1975 and 1976, the Satellite Instructional Television Experiment (SITE) reached 2,400 villages with educational programming, demonstrating the potential of satellite technology for social development. This experiment, conducted using NASA’s ATS-6 satellite, became the foundation for future satellite-based health applications.

ISRO made a modest beginning in telemedicine with a Telemedicine Pilot Project in 2001, connecting Chennai’s Apollo Hospital with a rural hospital in Aragonda village, Andhra Pradesh. This initiative expanded rapidly, linking remote areas in Jammu, Kashmir, Ladakh, the Andaman and Lakshadweep islands, and northeastern states to specialty healthcare centers.

National telemedicine networks

The success of pilot projects led to establishing larger networks. The National Medical College Network (NMCN) interconnects 150 government medical colleges for e-education and e-healthcare delivery using the National Knowledge Network’s high-speed connectivity. This network includes a National Resource Centre at AIIMS New Delhi and eight Regional Resource Centres across the country, enabling knowledge sharing and specialist consultations across geographical boundaries.

e-Sanjeevani: India’s national telemedicine service

e-Sanjeevani is a first-of-its-kind telemedicine initiative by any country, developed indigenously by the Centre for Development of Advanced Computing (C-DAC), Mohali. This cloud-based platform operates in two variants: e-Sanjeevani AB-HWC for doctor-to-doctor consultations under the Ayushman Bharat Health and Wellness Centres scheme, and e-Sanjeevani OPD for direct patient-to-doctor consultations.

Over 163 million consultations were conducted across all 28 states and 8 union territories by September 2023, with the provider-assisted AB-HWC model accounting for more than 93% of usage. The platform has now served over 330 million patients. It operates through an impressive network of over 1.31 lakh health facilities as spokes, supported by nearly 17,000 hubs and over 680 online OPDs, with more than 2.3 lakh healthcare practitioners actively using the system.

This free-to-use National Telemedicine Service has provided over 276 million consultations and demonstrated promise in reducing systemic inequalities in healthcare access, particularly benefiting women and adults aged 25-45 who show the highest utilization rates.

CoWIN and Aarogya Setu: pandemic response platforms

The COVID-19 pandemic accelerated India’s digital health adoption significantly. Prime Minister Narendra Modi launched the COVID-19 Vaccine Intelligence Network (CoWIN) on January 16, 2021, serving functions of registration, appointment scheduling, identity verification, vaccination, and certification.

CoWIN leveraged existing digital assets including the Electronic Vaccine Intelligence Network (eVIN) for supply chain management, Digital Infrastructure for Vaccination Open Credentialing (DIVOC) for certificates, and the Surveillance and Action for Events Following Vaccination (SAFE-VAC) system. With over 940 million registered users, the platform enabled vaccination of over 25 million people in a single day, demonstrating unprecedented scalability.

The Aarogya Setu mobile application complemented CoWIN by enabling contact tracing and health status sharing during the pandemic. Together, these platforms showcased India’s capability to rapidly deploy digital health solutions at population scale.

Ayushman Bharat: comprehensive health coverage

The Ayushman Bharat programme represents India’s most ambitious healthcare initiative, operating through two interconnected components. The Pradhan Mantri Jan Arogya Yojana (PM-JAY) provides health insurance coverage of up to Rs 5 lakh per family annually for secondary and tertiary hospitalization. The experience of AB-PMJAY in using public digital infrastructure has been leveraged to expand the reach of digital health services to all residents.

The second component involves transforming existing health facilities into Ayushman Bharat Health and Wellness Centres (AB-HWCs), now rebranded as Ayushman Arogya Mandirs. These centres deliver comprehensive primary healthcare services including teleconsultation through e-Sanjeevani integration, extending specialist care to rural populations.

Supporting digital infrastructure: HMIS, ABHA, and provider registries

Several complementary systems strengthen India’s digital health ecosystem. The Health Management Information System (HMIS) enables computerized registration and electronic health record capture in public health facilities from primary health centres upward. Various implementations including eHospital, eSushrut, and eUpchaar are operational across states.

The Healthcare Professionals Registry (HPR) creates unique identifiers for doctors across the country, while the Health Facility Registry (HFR) maintains standardized information about healthcare establishments. By November 2023, 150 digital health initiatives had graduated from the ABDM Sandbox, achieving full compliance and integration with the national digital health infrastructure.

The Online Registration System (ORS) has been implemented in over 748 hospitals, facilitating online registration, fee payment, appointment booking, and access to diagnostic reports, with more than 99 lakh online appointments conducted as of December 2024.

Emerging frontiers: AI and future directions

Artificial intelligence is being integrated into India’s digital health ecosystem. The eHealth AI unit of MoHFW, operated in collaboration with the Wadhwani Institute of Artificial Intelligence, is developing image-based AI solutions with premier institutions including AIIMS Delhi, AIIMS Rishikesh, and PGIMER Chandigarh. Clinical Decision Support Systems have been deployed within e-Sanjeevani nationally, while AI solutions for diabetic retinopathy screening and chest X-ray analysis are operational at AIIMS Delhi.

The Health Ministry is working on upgrading CoWIN for India’s universal immunization programme, allowing healthcare professionals to digitally track immunization status of mothers and children in real-time. This evolution from pandemic response to routine healthcare demonstrates the sustainable value of digital health investments.

Challenges and the road ahead

Despite remarkable progress, challenges remain. Digital literacy gaps, particularly in rural areas, affect adoption rates. Studies indicate that awareness about teleconsultation services remains low in some rural communities, with unfamiliarity being the primary barrier to usage. Ensuring interoperability across diverse state-level systems, maintaining data privacy and security, and building healthcare worker capacity for digital tools require continued attention.

India’s participation in global digital health forums, including chairing the Global Digital Health Partnership in 2020 and leading the BRICS Digital Health Summit in 2021, positions the country as a leader in sharing lessons from its ambitious digital health transformation with the world.
